About the Change:

On January 1, 2026 Centers Plan for Medicaid Advantage Plus (HMO D-SNP) (“Centers Plan”) members will transition to Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP) (“Plan”) offered by Anthem Blue Cross and Blue Shield ("Anthem").

Members: What to expect

  • What do I need to know?
    You will become a member of Anthem, unless you choose to transfer into a different MAP Plan.  Please note that even though our name will change to Anthem, you will continue to receive the same excellent care.  Your services, home care assistants, and care management team will remain the same.  Only the name will change.
  • If you want to stay with Anthem, you do not need to do anything.
    If you do not wish to transition to Anthem, you can transfer to a plan of your choice by contacting New York Medicaid Choice at 1-888-401-6582 as soon as possible but no later than December 31, 2025. 
  • What are my options?
    – You can receive your long-term care services from Anthem.  If you choose this option, you do not have to do anything.  You will be covered by Anthem for your long-term care services effective January 1, 2026.

    – You can receive your long-term care services from another managed long term care plan.  For more information about other managed long-term care plans available to you please call New York Medicaid Choice at 1-888-401-6582. 

  • If you do not select an alternate MAP Plan by December 31, 2025, you will be covered by Anthem for your MAP services effective January 1, 2026.  
    Centers Plan and Anthem will work to ensure that the transfer of your membership to Anthem is smooth and without disruption. 

Dental Benefits: What to expect

  • What do I need to know?
    LIBERTY Dental Plan is replacing DentaQuest as our dental vendor.  Starting 1/1/2026, you will be receiving your dental benefits from LIBERTY Dental Plan.
  • What to expect?
    Your benefits are not changing, but some of the dentists in our plan might.
    If your current dentist is no longer in our plan, you may need to switch to a new dentist.

    You will need to call your provider to make sure that they are in our network before making an appointment. 

  • How to find a new dentist?
    To see which dentists are in our plan:
    – Visit www.libertydentalplan.com
    – Call LIBERTY Dental Plan at 1-888-700-0992 (TTY 711). They are happy to help you find a new dentist close to your home if you need.
     
  • What happens to the services I am receiving now?  
    If you are receiving ongoing care from a DentaQuest dentist, please contact LIBERTY Dental Plan at 1-888-700-0992 (TTY 711) to let them know.  They will work to make sure you can continue your care. 

Vision Benefits: What to expect

  • What do I need to know?
    Superior Vision is replacing VSP as our vision (i.e., eye care) vendor.  Starting 1/1/2026, you will be receiving your vision benefits from Superior Vision.
  • What does this mean?

    Your benefits are not changing, but some of the vision providers in our plan might.
    If your current vision provider is no longer in our plan, you may need to switch to a new vision provider.
    You will need to call your provider to make sure that they are in our network before making an appointment. 

  • How to find a new eye doctor?
    To see which vision providers are in our plan:
    – Visit www.superiorvision.com
    – Call Superior Vision at 1-800-879-6901 (TTY 711).  They are happy to help you find a new vision provider close to your home if you need.
     
  • What happens to the services I am receiving now?  
    If you are receiving ongoing care from a VSP vision provider, please contact Superior Vision at 1-800-879-6901 (TTY 711) to let them know.  They will work to make sure you can continue your care.

This change in ownership won’t impact your 2025 coverage. You will still get the same comprehensive care and service, from the same providers, as you do now.
