Elevance Health, the parent company of Anthem HP, LLC (d/b/a Anthem Blue Cross and Blue Shield HP) (“Anthem”) and Wellpoint of NY, LLC, acquired Centers Plan on January 1, 2025.  As part of the transition, certain product lines were transferred as follows: 

• MAP Transition Date: Effective January 1, 2026, members previously enrolled in Centers Plan for Medicaid Advantage Plus (“MAP”) were enrolled in Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP), unless they selected another MAP plan.

• MLTC Transition Date: Effective March 1, 2026, members previously enrolled in Centers Plan’s Managed Long-Term Care (“MLTC”) plan living in Bronx, Kings, Nassau, New York, Queens, Richmond, Rockland, Suffolk, and Westchester counties were enrolled in Anthem, unless they selected another MLTC plan.  Members living in Erie and Niagara counties, were enrolled in Wellpoint of NY, LLC, unless they selected another MLTC plan.
